Vendor needs to provide employment medical services with physical examination within five days of referral and provide the written follow-up evaluation report to the government authority located in Hartford, CT.
- A comprehensive medical history must be obtained from each potential employee.
- This medical history should include, but is not limited to the following information: dates of prior physical examinations, immunization history.
- The physical examination must be conducted in accordance with sound medical
practice and shall include, but is not limited to the following: height, weight, pulse, blood pressure, respiratory rate, gross hearing screening.
- The evaluation report must be provided to agency administration via U.S. mail or by facsimile within ten days of the examination. All other records obtained during the course of the examination shall be maintained by the service provider unless otherwise directed by the Judicial Branch.
- The results of the drug and alcohol screening whether an acknowledgement of physical defect was presented to the applicant for presentation to the commissioner of workers compensation and the nature of the defect.
