[*] Scope of Service:

Vendor needs to provide professional screening, counseling, treatment services, electronic monitoring, and remote alcohol testing services to the government authority located in Peoria, AZ.
- Customers are referred and directed by the court to register and complete counseling programs on an as-required basis.
- The customer is liable for all payment to the contractor.
- The services will be provided on an as-required basis and the City does not guarantee a minimum amount of Services during the term of this Agreement.
- The customer record shall be maintained alphabetically, by last name. The customer record shall include the customer signed invoice of services and price sheet.
- Contractor shall have the ability to conduct intake screening and treatment classes at the contractor’s facility.
- Contractor must maintain all current state and city licenses for the operation of its business throughout the term of the agreement and shall provide proof of licensing and appropriate certification upon request.
- Contractor shall provide each customer with a single invoice listing separately all services and their individual cost in accordance with the price sheet.
