[*] Scope of Service:

(1) Vendor needs to provide transportation assistance services to the government authority located in Missouri.  
- Home to door service as necessary to assist the rider from the home to the vehicle and through the door of the destination.
- Administration, detailed billing, and record keeping services associated with the program including a quarterly database update on available trips.  Each quarter, participant accounts will be updated with new trips.
- Transportation on holidays, public health emergency, and disaster declarations for life-sustaining medical appointments including dialysis, radiation and chemotherapy or any other life sustaining appointment as agreed upon by the agency and provider.
- Hours of operation should be from 6am to 6pm monday through saturday.
- Participants will be asked to give minimum 24 hours’ notice for scheduled trips.  Accommodating requests with less than 24 hours’ notice is at provider’s discretion as long as all participants are treated equally.
(2) All questions must be submitted no later than September 21, 2023.  
(3) The contract term will be one year.
